Intermittent skilled nursing services:

After receiving a referral from the provider, the nurse will visit the patient in his/her place of residence. During the first visit, the RN will perform a comprehensive evaluation to assess the patient's needs, provide a plan of care or a service plan that specifies a level of care the nurse will follow.

Patient/Caregiver Health Education:

  • Diet and Exercise

Speech Therapy

Our speech therapists (STs) develop rehabilitative plans of care, collaborate with the health care providers to obtain orders, provide education to patients and families and set achievable goals to discharge. The ST provides all care in the patient's home.

Occupational Therapy

Our occupational therapists (OTs) visit a patient in their home and, after an evaluation, create a care plan that offers strategies for carrying out the patient's daily activities. An OT finds the right fit between the patient's skills, the necessary and desired activities, and the environment of the patient's home so that the patient can safely manage while at home.

Physical Therapy

Our physical therapists (PTs) are movement experts who can identify, diagnose, and treat physical problems. The PT Performs a physical assessment, provides a plan of care and treats the patient in their own homes, which allows the PT to provide functional treatments aimed at maximum independence in the home. Treament plans are designed for each person’s individual goals, challenges, and needs.

Home Health Aide

Our home health aides (HHAs) assist with activities of daily living:

  • Bathing/Personal Care
  • Ambulation and Exercise
  • Meal Preparation
  • Medication Reminders
